Youth Liability Form: If you have a student that will be going on an off campus activity with us or will be transported in the church van then we ask you to update your information with us by filling out a new liability for each year. Please access the form by Clicking Here

Once you hit submit, the form will be sent directly to Jonathan and saved on file for 2023. This will save time signing up for upcoming youth events.
